• Sport registration cannot be completed without a valid physical on file.
• Physicals are good for three years. A copy of a visit summary does not qualify as a sports physical, a completed MSHSL sport physical form (or clinic specific letter) signed by your provider is required.

• Free/Reduced Benefits - Athletes should apply for benefits prior to registering.

• All non-athletic activity and sport fees (with the exceptions of summer strength training & clay target range fees) count toward the annual family cap of $625.
